To understand why these bug occurs, we need to understand where is the problem. Go to your SkyrimEditor.ini file located in your Skyrim directory. There are 3 things required to change an NPC appearance: 1, ESP entry for NPC adjustments, which you can see in Creation Kit or TES5Edit. I am going to submit a bug report with all of the things i have discovered trying to get this to work in MO2. Skyrim change npc appearance creation kit Make New Folder and naming it Creation Kit.
